We are committed to providing medical care that is affordable for every family. Please feel free to inquire about the costs associated with your treatment. We will work with you and your insurance company to provide you with as accurate cost information as possible. Payment and/or insurance co-pays are due when services are rendered.
We accept Visa, MasterCard, Discover and offer a discount to patients without insurance coverage.
If special arrangements are needed, please speak with our office manager prior to receiving services.
Our office staff will be glad to assist you with your insurance coverage. Each time you visit our office, bring your current insurance card with you. If you change insurance companies or join another employer’s plan, please be sure to let our Billing Department know.
We normally submit all insurance claims for you. We are committed to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 45 days. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance claim form.
